Dr. Forman graduated from NYU College of Dentistry in 1977. He served as a dental officer in the United States Air Force, practicing in all areas of general dentistry. He currently has been serving the greater Mukilteo area, at his office in Harbour Pointe, since 1997.
Since he began practicing dentistry, Dr. Forman has kept up to date on continuing education. In 1997, Dr. Forman was awarded the distinguished honor of Fellowship in the Academy of General Dentistry. This award is given to member dentists who have devoted over 500 hours of study in advanced continuing education in dentistry and have passed a rigorous written exam. This demonstrates his commitment to the practice of general dentistry.

Dr. Forman resides in Mukilteo with his wife and son. He enjoys spending time outdoors exploring the Pacific Northwest.
