Hours:
Our office hours are flexible, and we try to accommodate each patient. Appointments can be made as early as 7:45am and as late as 5:00pm Monday Tuesday, and Wednesday, and as early 7:45am and as late as 12:00pm on Thursday. Our office is closed Friday, Saturday, and Sunday. Dr. Forman can always be contacted for after hours emergencies via telephone voice message.

We take great pride in our clean and warm environment. We use state of the art sterilization procedures to ensure patient safety. In addition to our comprehensive dental care, you will be treated in an ultra-modern dental office utilizing many of today's latest dental technologies.

Insurance
We will make every effort to help you maximize your insurance benefits, and will submit your claim for you. We accept most insurance plans including federal government employees, active duty military dependents and retirees.

Cancellation
To better serve all of our patients, we kindly request a 24 hour notice of appointment cancellations.
Financial
If you have dental insurance, any co-payment is due at time of service. If you do not have dental insurance, payment in full is expected at time of service. A 10 percent discount is offered to those without insurance that pay with cash or check. For your convenience, we also gladly accept VISA or MasterCard. We are also glad to offer Care Credit for those patients who wish to finance the cost of their dental treatment. You may find out more information and apply for credit at www.carecredit.com.
